signed and dated declaration form, 2 passport size photographs with a white background and the appropiate
cheque to our P.O.BOX Address in Southall

| IMPORTANT NOTE: |
- Application form must be filled directly online
- Print out
the completed form and please make sure it is signed and dated, before posting to us
- Misleading of facts or providing false information may lead to refusal of visa without any conveying cause for that
- Attach photographs with plain white or light backgrounds only
- Make sure you don’t posses a visa when you fill your new
visa applications form and VISA fee is not-refundable once it is put forward.
